Using expedited services to get a new passport has its rewards. So, getting your child’s passport expedited is no different. With expedited services, you will get a passport much faster than the average routine processing time. To use this service, you must follow some procedures. Here they are:
- Your child is a US citizen
- Your child is 15 years or younger
- You have proof of travel in two weeks or less
Once you can satisfy the above criteria, you can proceed to complete any forms necessary for the application and gather all your supporting documents for submission. Complete Form DS-11 Application for a New US Passport. This form will provide all the information about the applicant. Form DS-3053 Statement of Consent and Form DS-5525 Statement of Exigent/Special Family Circumstances may be necessary (these will be discussed in detail later).
Supporting documents will provide proof of relationship with the child and parent/guardian. Only original or certified copies of some documents are going to be accepted. Others must be a photocopy of the original document.

Form DS-11 Application for a New US Passport
Complete this form online and print it. DO NOT SIGN IT until you submit it to the expeditor.
Form DS-3053 Statement of Consent
The parent/guardian who cannot sign the child passport application must complete this form. He/she must get this form notarized; otherwise, it will not be valid. If both parents/guardians signed Form DS-11, this form is not needed.
Form DS-5525 Statement of Exigent/Special Family Circumstances
The parent/guardian applying for the child’s passport must complete this form if, and only if, the other parent/guardian cannot be reached or located to sign Form DS-11. In this circumstance, the applying parent must provide proof of absent parent/guardian. Evidence can be:
- Court document stating parent/guardian is in prison
- The police report of a protective order against the parent/guardian
- Absent parent is deceased
- The child got adopted by a single parent
- Absent parent is unknown
Supporting Documents
Along with the forms, supporting documents complete the application. These supporting documents include:
- Proof of US Citizenship (Birth Certificate, Certificate of Naturalization, Birth Abroad Certificate)
- One Passport Photo (2in x 2in, color with the white background, no glasses)
- Copy of Parent Valid ID (Driver’s license, State-issued ID, or Military ID)
- Two Letters of Authorization
